Appointments

At Chiswick Health Practice we really try to make appointments accessible and easy for everyone to book. We have a number of ways patients are able to book appointments or contact clinicians and the surgery.

How do I make an appointment?

Appointments in General

When booking a general appointment you will be offered a telephone/video consultation first. The doctor or nurse will then call you to determine if you need to attend the surgery physically

Appointments are booked through the week with those in most need booked on the day.

If your ailment is visual i.e. rash, lump etc the reception staff will ask you to either send a photograph via email to Chiswickhealth.practice@nhs.net . Please state your name and date of birth on the email.

Alternatively if you have chosen a video consultation appointment you will need to click a link to connect to a video consultation., when instructed to do so via text message by the doctor or nurse.

If following this they require you to attend the surgery physically they will organise the date and time to do so.

NOTE:

When booking for Immunisations and vaccinations /cervical screening /INR screening or requested bloods. These can be booked directly with the nurse or phlebotomist through reception and do not need an initial telephone or video consultation with a clinician.

Telephone Appointments

Calls are booked for the doctors and nurses through the week in:

  • AM Sessions between 9am and 12 noon
  • PM Sessions between 3pm and 6pm.

We are unable to give specific times during these sessions as some calls take longer than others and may require further contact with secondary care.

You will be asked for confirmation of contact details – preferable a landline – at the time of booking.

If you have a telephone call booked please have your landline or mobile with you and the volume turned up.

The call will come from an unknown number so please be aware.

If the clinician is unable to contact you after two attempts you will be informed that you need to rebook.

Video Consultations

Video calls are booked for the doctors/nurse through the week in:

  • AM Sessions between 9am and 12 noon
  • PM Sessions between 3pm and 6pm.

We are unable to give specific times during these sessions as some calls take longer than others and may require further contact with secondary care.

You will be sent confirmation with a link via your telephone or chosen device. You will need to follow the instructions regarding checking that sound and video is turned on and be available for your call.

Once your Clinician is ready they will send you a message inviting you to join the call.

If the clinician is unable to contact you after two attempts you will be informed that you need to rebook.

Online Booking of Appointments

Due to changes with regard to our appointment system due to the current pandemic we have made the decision to turn off the ability for patients to book their own appointments via the Patient Online system. Please be assured the patient online system is still working and available for patients to check test results and to access their medical records.

Face to Face Appointments

If you have been requested to attend the surgery for a Face to Face appointment or you are attending for Immunisations /Vaccinations /cervical screening /INR screening or requested bloods etc

Please phone the practice if since booking your appointment you have developed a cough, temperature, loss of taste or smell or have any concern that you might have Covid-19.

  • Please use toilet facilities at home/work prior to attending your appointment. There are no facilities operational in the Health Centre
    • Do not attend before your appointment time is due
    • Please attend alone unless accompanying a child or you are in need of assistance – please only bring one carer
    • Use the hand gel sanitiser on entry
  • Please wear a face covering at all times

 

  • Do not bring food or drink into the building
  • Do not bring leaflets/papers/magazines/toys
  • Please report to reception on arrival where you will be directed where to sit – areas have been designated and marked
  • Please keep personal belongings (keys, phones, jewellery) in your bag
  • If you have urinary symptoms bring a urine sample in a clean pot
  • Please use hand sanitiser when leaving the building

Clinicians will be wearing full “PPE” during your face to face consultation and all rooms are clean between each patient’s visits.
